Do not send as picture attachments, these will not be accepted. 

If you need a printed copy or would like to drop off once filled out you can stop at the Parks office located at
155 S Clute Park Dr Watkins Glen

Summer Recreation is open for children who are 5 years old & enrolled in Kindergarten through 5th grade.
